What to Bring for Medicaid Assistance:
– Parent’s/Adult’s Photo ID’s
– Child’s Birth Certificate, U.S. Passport, Hospital Record or Certificate of Naturalization
– Social Security Numbers for all Members
– Utility Bill, Driver’s License, Lease Agreement, or Letter from Landlord
– Paystubs for the last 60 days, Statement from your employer or self-employment records.
– If applicable: Resident Card (I-551), arrival/departure form (I-94), or papers from US Citizenship or Immigration Services
What to Bring for SNAP Assistance:
– Parent’s/Adult’s Photo ID’s
– Child’s Birth Certificate, U.S. Passport, Hospital Record or Certificate of Naturalization
– Social Security Numbers for all Members
